EXTENDED ACCESS APPOINTMENTS Evenings & Weekends (Stevenage Health Ltd)

NHS East and North Hertfordshire Clinical Commissioning Group are looking to improve access to primary care services, including making appointments available at times that are convenient to patients through the GP Extended Access service.

To achieve this we are working with our local GPs and have engaged with the patient population to develop a service that meets their needs. The extended access service is starting in Stevenage from July 2018 and will be provided by Stevenage Health Ltd the collaborative company formed by all Stevenage practices.

What is the GP Extended Access Service?

The Extended Access Service will make appointments with GPs in east and north Hertfordshire available at times that suit you, including evenings and weekends.

When are these appointments available?

Appointment times will be available in the evening and at weekends. The service will run from Kingsway Health Centre located on Stevenage leisure Park, 15 Kings Way, Stevenage SG1 2UA.

Who can access these appointments?

From July 2018, patients registered with 7 GP Practices in Stevenage will be able to access the service.
How do I make an appointment?
In the first instance we would advise patients to contact their individual GP practice for more information on their extended access service and how patients can make appointments.

What can I be seen for?

A GP Extended Access appointment is exactly like an appointment at your own GP practice. The service can see babies and children.

Will I see my own GP and/or Advance Nurse Practitioner (Minor Illness Nurse)?

You will be seen by local GPs and ANPs, but it is unlikely that you will be seen by your own GP or ANP. If you would prefer to see your usual GP or practice nurse, please book an appointment with your usual practice during their normal opening hours.

Will the GP I see have access to my medical record during my appointment?

Your medical record will, with your consent, be available to the GP/ANP that sees you, to ensure that they have the information they need to give you the best possible care. Notes of your appointment will be sent back to your own GP practice to ensure that your record is up-to-date. An appointment with this service will be just like an appointment at your own GP practice. Further information about how we use your medical record is available at on the CCG website here: www.enhertsccg.nhs.uk/mcr

For further information on extended access services please visit your own surgery website where regular updates will be available.

Making an Appointment

Non-urgent appointments may be made by telephoning 01438 742639 or by calling at the surgery during opening times Monday to Friday.

On-Line appointments are available for afternoons up to 4 weeks in advance with a GP or Nurse. These are for routine probems. Any appointments that are for longer duration should be made by calling the Surgery.

To use tne SystmOnline service, you will need to go to your GP practice to request a user name and password. You cannot register for this service online because your identity needs to be verified. Consider calling the practice in advance to find out what identification you will need to take with you.

Once a member of staff has given you a user name and a random password, wait an hour before trying to log in to the website. Your GP practice may have set up a link from their own practice website, which you can follow to access the Login page. Otherwise, your practice will have given you the web address you need to type into your web browser.

Enter your user name and password and click Login. You will then see the SystmOnline Home Page, which shows your details and your GP practice's contact details at the top.

You can navigate to all the online services from here, and can return to the Home Page by clicking Home in the blue menu bar, or by clicking the Back button on any page.

All appointments are for one person and one problem only.

If you cannot keep an appointment please inform us as soon as possible so someone else can benefit from the appointment.

Emergency Appointments

There will be appointments that can be booked on the day with both the Doctor and the Nurse Practitioner. Please telephone the Surgery as soon as possible on the day if you feel an appointment is of an urgent nature.

If you are concerned about your child, we will see them on the same day at the surgery where necessary.

Home Visits

Home visits are for housebound patients; lack of transport is not a reason for requesting a home visit. Whether or not the doctor will visit is at their discretion.

Please telephone the surgery before 10.00am whenever possible.
