For Patients who prefer to request your regular medication in person or by post please follow these guidelines:
Patients should complete the tear off slip from the right hand side of the prescription
Patients should tick the relevant medication that is required
If the medication is not on this slip please advise us.
If a tear off slip is not available patients should complete the manual form available from the surgery repeat prescription rack.
Prescription requests should be left in the black box on the wall in the surgery hallway.
Repeat prescription requests cannot be taken over the telephone as this blocks incoming calls that may be urgent and it can also lead to prescribing errors
Patients should allow 48 hours for collection and should be collected after 4pm.
If it is difficult to visit the surgery postal requests will be accepted and returned to the patient if a sae is enclosed with the request – please allow 1 week for return.
For patients who prefer to use our online request facility please follow these guidelines:
Logging in to SystmOnline
To use tne SystmOnline service, you will need to go to your GP practice to request a user name and password. You cannot register for this service online because your identity needs to be verified. Consider calling the practice in advance to find out what identification you will need to take with you.
Once a member of staff has given you a user name and a random password, wait an hour before trying to log in to the website. Your GP practice may have set up a link from their own practice website, which you can follow to access the Login page. Otherwise, your practice will have given you the web address you need to type into your web browser.
Enter your user name and password and click Login. You will then see the SystmOnline Home Page, which shows your details and your GP practice's contact details at the top.
Note: The services available here will depend on the services your GP practice has decided to offer.
You can navigate to all the online services from here, and can return to the Home Page by clicking Home in the blue menu bar, or by clicking the Back button on any page.
Please allow 48 hours for the collection of prescriptions order online. Collections will be ready from 4pm at the surgery.
TO LOG OUT
1. Click the Logout link on the Home Page or the Logout button on the main menu bar.
2. Close your Internet browser.
Note: The website will automatically log out after a period of inactivity. Type in your user name and password to re-enter the site.
General information regarding repeat prescriptions
It is the responsibility of the patient to advise the surgery if a medication is no longer required and the reason why so that records can be updated.
Please advise the receptionist if you need sychronisation of your repeat medication to enable one request each month for your full list of medication.
Please ask reception for details of local pharmacies that offer a courier service to the surgery.
The surgery follows the primary care trust 28 day prescribing policy – therefore only one months supply of medication can usually be given.
Please advise reception if you are going on holiday so that temporary adjustment to your supply of medication can be given to cover the holiday period.
Medication reviews will be undertaken every 6 months and patients should attend these to ensure future repeat prescription can be authorised.
